New Jersey now holds the top spot, but ranked 17th in the spring.

"For everyone in Catholic Health - from our physicians, nurses, clinical professionals and support staff, to our leadership and board of directors - quality and patient safety is our top priority", said Mark Sullivan, president and CEO of Catholic Health. Five Garden State facilities have received straight A's since the first Leapfrog survey, conducted in the spring of 2015; and four improved two letter-grades in six months alone.

"Leapfrog's Hospital Safety Grades recognize hospitals like Baylor St. Luke's Medical Center, CHI St. Luke's Health-The Woodlands Hospital and CHI St. Luke's Health Brazosport that focus on advancing patient safety".

Mercy hospitals are being recognized for protecting patients and meeting high safety standards.

New Jersey led the nation with the highest percentage of hospitals with an A grade in the latest fall Leapfrog Hospital Safety Grade released Thursday.

Of more than 2,600 hospitals graded nationwide, 32 percent earned an "A", 24 percent earned a "B", 37 percent a "C", 6 percent a "D" and just under 1 percent an "F".

"The hospital industry is most efficient in addressing patient safety when they work together, I think that is why when we see an improvement like in New Jersey it suggests that the whole hospital industry has been focused on this as a goal", Binder said.

Since 2012, Leapfrog has assigned an A, B, C, D or F Hospital Safety Grade to hospitals twice a year based on their performance in preventing accidents, injuries and errors to patients.

Highlights from the Leapfrog Grades for New Jersey include: 38 hospitals, or 56.7 percent, received an A grade; 16 hospitals, or 23.9 percent, got a B; 11 C hospitals, or 16.4 percent, got a C; one hospital received a D and another an F.

Saint Michael's CEO Robert Iannaccone said because of the precarious financial condition of the hospital prior to the sale to Prime Healthcare, the previous owners of Saint Michael's stopped submitting surveys to Leapfrog Group, which resulted in the failing grade.

"Transparent reporting on care quality is an absolutely critical tool to drive improvements", said state Department of Health Commissioner Dr. Shereef Elnahal. "This top grade is a result of our ongoing efforts to ensure patient safety and achieve the highest outcomes".

The safety score comes from data compiled by the Centers for Medicare & Medicaid Services, Agency for Healthcare Research Quality, Centers for Disease Control and Prevention, American Hospital Association, Health Information Technology Supplement and Leapfrog's own survey.

The Leapfrog Hospital Safety Grade uses 28 measures from public hospital safety data to assign grades to more than 2,600 USA hospitals.

"We send comments to the CMS every year on their proposed inpatient prospective payment system rule, and we say, 'you really should have some measures on medication safety.'
